In Raising Hell, although not seen, Gacy's soul was trapped in the Soul Catcher by Rowena MacLeod along with all of the other ghosts haunting Harlan. Shocked, Gacy complies and glares after Sam, Castiel and the two victims they rescued from him as they escape. Gacy screams at Sam in rage, causing Sam to order the psychotic clown to shut up. The ghosts catch up and Gacy tries to stab Sam, only to hit the barrier created by the spell which traps the ghosts within a one-mile radius. Discovering that he is out of ammo, Sam is forced to run as well, carrying the daughter to safety.

When Ethan went to wash his hands in the bathroom, Gacy terrorized the boy by fogging over the bathroom mirror, appearing in it when he wiped away the fog and then lunging out of the mirror at Ethan. Gacy's ghost was released from the box as a result. After the Impala was stolen by Max, the cigar box was taken to an abandoned house along with the rest of Terry's collection where the partying kids left the box unlatched. In Lebanon, after Terry's death, his cigar box was taken by hunters Sam and Dean Winchester who were unaware that Gacy's ghost was in the box. At some point, his cigar box fell into the hands of Terry. He was eventually captured and executed for his crimes, but his ghost became attached to his cigar box. In life, John Wayne Gacy was an infamous serial killer known as the Killer Clown who raped, tortured, and murdered at least thirty-three boys and young men.
